What counts as a "Stealth Kill?"

Hi,

I'm trying to level stealth, need 10 stealth kills.

Nothing I do seems to count. I am "not detected" when I do it. Even tried 1 hitting a pirate with my knife.

Sarah is with me. Is that the issue? Or something else?
